Depakote divalproex ; and Lamictal lamotrigine ; are the only two anticonvulsants approved by the FDA for treatment of seizure and bipolar disorders; the other anticonvulsants are indicated primarily for treatment of epilepsy. The use of a medication for its approved indication is called labeled use. In clinical practice, however, physicians often prescribe medications for unlabeled "off-label" ; uses when published clinical studies, case reports, or their own clinical experiences support the efficacy and safety of those treatments. Based on a number of clinical studies, some medications with anti-epileptic properties are also effective for treating bipolar disorder, despite the lack of an FDA-approved indication for this purpose. A number of clinical studies have shown that Tegretol carbamazepine ; , Depakote divalproex sodium ; , and Lamictal lamotrigine ; are effective for bipolar disorder. It is not totally clear why some anticonvulsants are effective for seizures and bipolar disorder. The anticonvulsants, which have very complex effects on the central nervous system, may be effective by controlling "kindling" in the areas of the brain from which the psychiatric disorder emanates. Kindling is a phenomenon that occurs when repeated subthreshold stimulation is applied to certain regions of the brain and sensitizes them, setting off a cascade of events leading to seizures or manic behavior. By decreasing electrical conduction or neurotransmitter activity in unstable brain cells, anticonvulsants are effective in controlling seizures and bipolar illness and toradol.

When we take the high frequency of overlapping symptoms of two commonly accepted and diagnosed conditions in child psychiatry, together with the one week duration requirement of mood episodes used to make the diagnosis by DSM-IV criteria, combined with the historical bias against diagnosing BPD before puberty, we need not search further for reasons as to why this condition is so poorly recognized and so often misdiagnosed in childhood. Since revisions of DSM-IV are not planned until after the year 2005, it behooves pediatricians, pediatric neurologists and child psychiatrists alike to become more aware of this area of diagnostic confusion and controversy. Risks of epilepsy and anticonvulsant therapy in pregnancy Maternal epilepsy is associated with a two-fold increase in the incidence of congenital anomalies in children born to women with epilepsy. Some of this increase in the incidence of congenital anomalies appears to occur even in the absence of anticonvulsant medication. There is even some evidence that paternal epilepsy is associated with an increased incidence of congenital abnormalities. Anticonvulsant therapy does appear to further increase the incidence of congenital abnormalities in children of epileptics, particularly with polypharmacy, but it is important to emphasize that some increased risk is present regardless of therapy when counseling epileptics about pregnancy. Despite the increased risk of congenital anomalies that may be associated with anticonvulsant therapy, it is generally accepted that anticonvulsive therapy is warranted during pregnancy because of the danger that both the potential physical trauma and the periods of maternal apnea associated with a grand mal seizure can present to the fetus.
